Preparing OB/GYN Ambient Documentation for 2027 CPT Maternity Care Code Revisions
Summary for Practice Administrators and CMOs. Read this block first, then use the playbook sections below.
The January 1, 2027 cutoff: The AMA FAQ (Sep 17, 2026) reports that the global maternity CPT codes are deleted. They are invalid for dates of service on or after January 1, 2027. The AMA counts 35 codes changed: 17 deleted, 12 new, and 6 revised.
The new reporting model: Antepartum and postpartum care are reported per visit using E/M services. Labor management (initial codes 59080/59081, plus subsequent-day codes) and delivery care receive their own phase-specific codes.
The new documentation risk: Under the global model, the bundle concealed thin per-visit notes. In 2027, every prenatal and postpartum encounter must stand on its own medical decision making (MDM). Labor and delivery attribution must also be explicit.
The operational fix: Ambient templates capture the encounter date, gestational age (GA), delivering clinician, and postpartum visit data as discrete fields. The capture happens at the point of care, not during a retrospective billing reconstruction.

The global obstetric package is ending. For three decades it let practices bill a single code for prenatal care, delivery, and postpartum care. Starting January 1, 2027, those services are reported separately, and each encounter must justify itself. Scribing.io wrote this playbook for the practice administrators and CMOs who will own revenue integrity during that transition.
This playbook covers operations. It is not a coding manual. It explains how Scribing.io's Ambient Clinical Intelligence, delivered through the Merry AI Clinical Interface, turns each prenatal and postpartum conversation into discrete, attributable, and countable data. Read it alongside the other guides in the Specialty Clinical Playbook Library and the Scribing.io specialties hub.

What Changes on January 1, 2027
The AMA published the revisions early. It released the maternity care revisions well ahead of the effective date. The goal was to give EHR vendors, payers, and practices a full year to rebuild charge capture. The AMA FAQ of September 17, 2026 is the primary reference, and the CPT 2027 code set is the controlling source.
Legacy global model versus the 2027 phase-specific model | ||
Element | Legacy Global Model (through 12/31/2026) | 2027 Phase-Specific Model |
|---|---|---|
Antepartum care | Bundled into global codes (e.g., 59400, 59510, 59610, 59618), or into package codes by visit count | Reported per visit using E/M services; each visit needs its own documented MDM |
Labor management | Bundled into the delivery or global code | New initial codes 59080/59081, plus subsequent-day codes [VERIFY numbers against CPT 2027] |
Delivery | Global, or "delivery only" codes | New and revised delivery care codes [VERIFY descriptors] |
Postpartum care | Bundled, or reported with 59430 | Reported per visit using E/M; window and content must be documented |
Attribution | Implicit to the billing provider on the global claim | Explicit per phase: who managed labor, who delivered, who saw each visit |
Code volume | N/A | 35 codes changed per the AMA: 17 deleted, 12 new, 6 revised |
The regulatory timeline in 2026:
RUC submission, February 2026: Valuation recommendations for the new codes went to CMS.
CY2027 MPFS Proposed Rule: Released July 14, 2026, with a 60-day comment period.
CY2027 MPFS Final Rule: Expected in early November 2026. It sets the final RVUs and payment policy.
CMS transmittals and change requests: [VERIFY at cms.gov before citing any transmittal or CR number.] This playbook does not cite unverified identifiers.
Date of service governs selection. A pregnancy that spans the cutover will have pre-2027 antepartum visits and 2027 delivery and postpartum services. Ask each payer for written guidance on split-pregnancy billing before December 2026.
Why Per-Visit Reporting Exposes Weak Prenatal Notes
The bundle masked thin notes. Under the global model, a note reading "routine OB, FHT 148, no complaints" rarely triggered an audit. That note was not supporting a separate charge. In 2027 the same note becomes the sole support for an E/M claim, and it will not survive review.
Three exposure points now appear:
MDM must stand alone: Every antepartum E/M needs a documented problem, data review, and risk level under current AMA E/M guidelines.
Counts must be reconstructable: Payers and quality programs will compare visit counts against claims. Missing encounter dates or missing GA create mismatches.
Attribution must be explicit: In a group, the clinician who managed labor may differ from the delivering clinician. Both may differ from the clinicians who saw the antepartum visits.
Cloned notes create new liability. Copying the prior visit's prenatal note forward was a common efficiency step under the global model. When each visit is separately billed, cloned documentation invites medical necessity denials and compliance scrutiny. Ambient capture produces a fresh, encounter-specific record every visit.
Clinical Logic: How a 14-Provider OB Group Runs Merry AI
The scenario below is verbatim. It is the reference workflow this playbook uses to test operational readiness. Each step maps to a discrete data element that the 2027 codes depend on.
A 14‑provider OB group preparing for 2027 runs Merry AI during every prenatal visit.
The ambient capture extracts visit date, GA, BP/weight, risk flags, and clinician ID; the Chrome extension injects these as discrete rows in the OB flowsheet and maintains a running antepartum visit ledger.
At delivery, Merry AI compiles the delivering clinician NPI and total qualifying antepartum visits and emits a CPT‑ready summary.
The postpartum visit is auto‑scheduled and documented within the computed 7–84‑day window, with the extension injecting the postpartum exam checklist.
Billing exports a structured JSON/CSV crosswalk for the new maternity care elements—no EHR API, no cloned notes, and denials drop because counts, GA, and clinician attribution are discretely present.
Why this design holds up under audit
Ambient templates capture discrete data. They record antepartum encounter dates, GA, the delivering clinician, and postpartum visit data, which are the elements the AMA's early-published 2027 CPT maternity revisions require. Because the data is captured as rows rather than narrative, nobody has to reconstruct it at delivery. The billing team reads a ledger instead of combing through 12 notes.
No EHR API is required. The Chrome extension writes into the existing OB flowsheet through the clinician's authenticated browser session. This shortens deployment for groups on EHRs whose vendors charge for, or delay, API access. See the EHR Clinical Integration Directory and the Scribing.io integration hub for EHR-specific notes.
Documentation workflow comparison for 2027 maternity reporting | |||
Capability | Manual Free-Text Note | EHR Macro or Template | Merry AI Ambient + Discrete Injection |
|---|---|---|---|
Encounter-specific MDM | Depends on clinician time | Prone to cloned defaults | Generated from the actual conversation; clinician-signed |
GA per visit | Often missing or inconsistent | Manual entry | Computed from EDD and injected as a discrete row |
Running visit count | Reconstructed at delivery | Requires a report build | Antepartum ledger maintained in real time |
Delivering clinician NPI | Inferred from the op note | Inferred | Captured and compiled into the CPT-ready summary |
Postpartum window tracking | Front-desk dependent | Scheduling rule, if configured | Computed 7–84 day window; checklist injected |
Billing export | Coder abstraction | Custom report | Structured JSON/CSV crosswalk |
The Discrete Field Architecture Behind CPT-Ready Summaries
Discrete beats narrative for billing. A narrative note tells the story of the visit. A discrete row lets a claim scrubber count visits, check dates, and verify attribution without anyone reading prose. The 2027 model rewards practices that produce both.
Field capture map by phase of maternity care | |||
Phase | Discrete Field | Source in Ambient Capture | 2027 Use |
|---|---|---|---|
Antepartum | Date of service | Encounter timestamp | Visit count; split-pregnancy logic |
Antepartum | Gestational age (weeks + days) | EDD on file, cross-checked against spoken GA | Trimester-specific ICD-10-CM; clinical appropriateness |
Antepartum | BP, weight, fundal height, FHT | Spoken vitals and exam findings | Data element of MDM; hypertensive disorder surveillance |
Antepartum | Risk flags | Problem discussion (e.g., GDM, cHTN, prior CS) | MDM problem complexity; O09.- selection |
Antepartum | Rendering clinician ID | Logged-in user, plus spoken attestation | Per-visit attribution |
Labor | Admission time; ROM timestamp | L&D dictation | Initial labor management code selection [VERIFY criteria] |
Delivery | Delivering clinician NPI; delivery date and time; route | Delivery note capture | Delivery code attribution |
Postpartum | Visit date; days since delivery; checklist completion | Postpartum encounter | 7–84 day window compliance; Z39.2 |
GA calculation is deterministic. Merry AI computes GA as 280 days minus the days remaining to the EDD on file, expressed in weeks and days. If the spoken GA differs from the computed value by more than a configured tolerance, the extension flags the
